Chronic Disease Management/Primary Care

Patients with chronic medical conditions will receive comprehensive evaluations that include obtaining a medical history, performing a physical examination, reviewing medications, and developing an individualized treatment plan. Vital signs will be obtained using battery-powered equipment, including a blood pressure monitor, thermometer, pulse oximeter, and digital scale. Prescriptions will be transmitted electronically through a secure electronic medical record system.
